Report of Birth
All children born in New Zealand, the Cook Islands and Niue to at least one Filipino parent at the time of birth must be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy in Wellington.
Report of Birth (ROB) applications may be submitted personally at the Philippine Embassy in Wellington or via courier/mail.
Important Note:
If filing via courier/mail, please email one (1) set of scanned copies of requirements (ONE FILE PER DOCUMENT) at: rob@philembassy.org.nz for pre-checking before the notarization and before sending your documents to the Embassy.
HOW TO FILE THE REPORT OF BIRTH
A. Prepare Your Documents. Complete the required documents by downloading the following required forms:
- Consular Service Slip Form - Accomplish 1 original only
- Report of Birth Form - Accomplish 4 originals. Must be typewritten with handwritten signature of informant, and notarized.
-Handwritten form will not be accepted. - Birth Certificate of Child (issued by the country of birth)
- 1 original and 4 photocopies - PSA-issued Certificate of Live Birth of Father *
- 1 original and 4 photocopies
(* Birth Certificate for non-Filipino, issued by the country of birth) - Passport (valid) Data Page of Father
- 4 photocopies only (do not send original passport) - PSA-issued Certificate of Live Birth of Mother *
- 1 original and 4 photocopies
(* Birth Certificate for non-Filipino, issued by the country of birth) - Passport (valid) Data Page of Mother
- 4 photocopies only (do not send original passport) - PSA-issued Marriage Certificate or PSA-issued Report of Marriage if married outside the Philippines
- 1 original and 4 photocopies

If married in New Zealand or outside the Philippines and parents’ marriage was not reported yet, you may file the Report of Marriage at the same time with your child’s Report of Birth - Plastic Courier Bag
- With barcode tracking number, size should fit an A4 bond paper; may be purchased from your preferred courier/postal service provider or any Post shop
- This will be used for the return of all submitted original documentary requirements and the approved Report of Birth

Late Registration of Birth (Child more that 1 year old)
- Affidavit of Late/Delayed Registration of Birth
- 1 original and 3 photocopies.
- Executed by the mother, father or legal guardian.
- Notarized by the Embassy, Honorary Consulate or Notary Public.

- 1 original and 3 photocopies
- Executed by 2 persons, personally known to the child's parents (not relatives) attesting the birth circumstances or facts surrounding the child's birth.
- Notarized by the Embassy, Honorary Consulate or Notary Public - Negative Certification/e-Certification of Birth Record (CRS Form no. 1) with QR Code, if child is more than 5 years old.
- Get this from the Philippine Statistics Authority (PSA) by ordering a Birth Certificate either personally, through an authorized representative in the Philippines, or online channel here: https://www.psaserbilis.com.ph/ or https://psahelpline.ph/. Online requests may be delivered anywhere in the Philippines or abroad, including New Zealand.
Unmarried Parents (Child’s Surname Using Father’s Surname)
If the father acknowledges his paternity, allowing the child to use his surname.
- Affidavit of Acknowledgement of Paternity (AAP) Accomplish Four (4) originals.
- Executed by the father
- Notarized by the Embassy, Honorary Consulate or Notary Public. - Affidavit to Use the Surname of the Father (AUSF)
- Accomplish Four (4) originals
- Executed by the mother, legal guardian or child (of legal age).
- Notarized by the Embassy, Honorary Consulate or Notary Public. - Sworn Attestation (This is needed only if the above AUSF is executed by the legal guardian or child of legal age)
- Accomplish Four (4) originals
- Executed by the mother
- Notarized by the Embassy, Honorary Consulate or Notary Public. (https://www.philembassy.org.nz/services/download-forms) - Certificate of Registration for the AAP and AUSF
- To be prepared by the Embassy/Consulate/Local Civil Registrar where AAP and AUSF were executed.
Children Included in Parent’s Dual Citizenship Petition
- Philippine dual citizenship Identification Certificate of Child
- Four (4) photocopies - Philippine dual citizenship Identification Certificate, Oath of Allegiance or Order of approval of Parent/s
- Four (4) photocopies
B. Email one (1) set of the required documents to rob@philembassy.org.nz for pre-checking.
C. Once the documents have been pre-checked, have the ROB forms and Affidavits (if applicable) notarized by the Philippine Honorary Consulate or Notary Public. Please set a schedule here if you wish to have your documents notarized by the Philippine Honorary Consulate (Auckland and Christchurch)
(The Embassy does not accept notarization from the Justice of Peace)

REMINDERS:
Processing time varies between 4 to 6 weeks depending on the volume of ROBs received by the Embassy.
The Embassy reserves the right to require additional documents that may be deemed necessary to prove citizenship, status or identity.
