Requirements for Report of Birth

  1. Kindly send the duly accomplished Report of Birth form and other required documents via courier to the Philippine Embassy with address at: 50 Hobson Street, Thorndon, Wellington 6011;
  2. All forms must be written or typed and printed in BLACK INK
  3. Civil Registry and Legalization Application Form (https://www.philembassy.org.nz/download) - 1
  4. Report of Birth Form (https://www.philembassy.org.nz/download) - 4 Originals
  5. Birth Certificate of Child – Original and 4 copies
  6. PSA or NSO marriage contract or, if married in New Zealand, Cook Islands, Fiji, Samoa and Tonga, Report of Marriage – Original and 4 copies
  7. Birth Certificate of Parents (PSA or NSO for Filipino Nationals) – Original and 4 copies
  8. Passport data page (both parents) – 4 copies (do not send original passport)
  9. Affidavit of late registration of birth (notarized by notary public/ PHL Embassy) if child was born more than a year ago – Original and 3 copies;
  10. Affidavit of two disinterested persons (notarized by notary public/ PHL Embassy) if child was born more than a year ago – Original and 3 copies;
  11. Payment: Bank account details of the Embassy will be provided after assessment of documents submitted. Please refer to our consular fees (https://www.philembassy.org.nz/consular/schedule-of-fees)
  12. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/ International courier if residing outside NZ)
  13. Any documents that may be required by the Philippine Embassy

Additional Requirement for Children of Unmarried Parents

  1. All requirements for Report of Birth (please see above)
  2. Affidavit of Acknowledgement of Paternity (AAP), to be executed by the father (https://www.philembassy.org.nz/download) – 4 original, to be notarized by the embassy (if acknowledged by father)
  3. Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother, legal guardian or child (if of legal age) (https://www.philembassy.org.nz/download#civilregistry) if the child is using the surname of the father – 4 originals, to be notarized by the embassy;
  4. Sworn Attestation to be executed by the mother (ONLY IF AUSF IS NOT EXECUTED BY MOTHER) (https://www.philembassy.org.nz/download) – 4 originals, to be notarized by the embassy;
  5. Certificate of Recognition to be executed by the embassy
  6. Please call the Embassy at 04-8903741 for fees regarding additional documentary requirement for unmarried parents

Requirements for Report of Marriage

  5. Marriage Certificate / Particulars of Marriage – original and 4 copies
  6. Birth certificate of both husband and wife (PSA or NSO for Filipino Nationals) – original and 4 copies;
  7. Passport data page (husband and wife) – 4 copies (do not send original passport);
  8. Affidavit of Late Registration of Marriage (stamp by notary public/ PHL Embassy/ Consulate) – original and 3 copies if marriage was contracted more than a year ago;
  9. Payment: Bank account details of the Embassy will be provided after assessment of documents submitted. Please refer to our consular fees (https://www.philembassy.org.nz/consular/schedule-of-fees)
  10. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/ International courier if residing outside NZ)
  11. In case of death of previous spouse, PSA or NSO death certificate or, for foreign national, local death certificate of previous spouse;
  12. PSA or NSO marriage contract with annotation of nullity of marriage for Filipino spouse with previous marriage;
  13. Court Order affirming the dissolution of marriage (divorce) for Foreign spouse with previous marriage;
  14. Any documents that may be required by the Philippine Embassy

Requirements for Report of Death

  1. By appointment (https://booking.philembassy.org.nz/), if submitted personally (please bring email or print out confirmation) or by courier/mail
  2. All forms must be written or typed and printed in BLACK INK
  3. Civil Registry and Legalization Application Form (https://www.philembassy.org.nz/download) - 1
  4. Report of Death Form (https://www.philembassy.org.nz/download) - 4 originals
  5. Death Certificate - original and 4 copies
  6. Medical certificate - original and 4 copies
  7. Certificate of No Infectious Disease – 4 copies
  8. If cremated, Certificate of Cremation – 4 copies
  9. Passport – 4 copies
  10. Payment should be cash or in bank check (payable to Philippine embassy). Please refer to our consular fees (https://www.philembassy.org.nz/consular/schedule-of-fees)
  11. Courier Bag with a “Courier Signature Required” sticker (within NZ if residing in NZ/ International courier if residing outside NZ)

Requirements for Consular Mortuary Certificate:

  1. Death Certificate - original and 1 copy
  2. Medical certificate - original and 1 copy
  3. Certificate of No Infectious Disease – 1 copy
  4. If cremated, Certificate of Cremation – 1 copy
  5. Passport – 1 copy
  6. Flight details
  7. Name and address of consignee
  8. Payment should be cash or in bank check (payable to Philippine embassy). Please refer to our consular fees (https://www.philembassy.org.nz/consular/schedule-of-fees)
