Requirements for Report of Birth

  1. By appointment (https://philembassynzl.appointy.com), if submitted personally (please bring email or print out confirmation) or by courier/mail
  2. Report of Birth Form (https://philembassy.org.nz/download#civilregistry) - 4 Original
  3. Birth Certificate of Child – Original and 4 copies
  4. PSA or NSO marriage contract or Report of Marriage – original and 4 copies
  5. Birth Certificate of Parents (PSA or NSO for Filipino Nationals) – original and 4copies
  6. Passport (parents) – 4 copies
  7. Affidavit of late registration of birth (stamp bynotary public/ PHL Embassy/ Consulate) – original and 3 copies if child was born more than a year ago
  8. Affidavit of two disinterested persons (stamp by notary public/ PHL Embassy/ Consulate) – original and 3 copies if child was born more than a year ago
  9. Payment should be cash/bank check (payable to Philippine embassy) or please refer to our consular fees (https://philembassy.org.nz/consular/schedule-of-fees)
  10. Prepaid courier bag signature required (within NZ if Residing in NZ/ International Courier if outside NZ)
  11. Any documents that may be required by the Philippine Embassy

For children of unmarried parents:

  1. All requirements for Report of Birth (please see above)
  2. Affidavit of Acknowledgement of Paternity (AAP), to be executed by the father (https://philembassy.org.nz/download#civilregistry) – 4 original, to be notarized by the embassy or consulate (if acknowledged by father)
  3. Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother, legal guardian or child (if of legal age) (https://philembassy.org.nz/download#civilregistry) – 4 original, to be notarized by the embassy or consulate (if using the father’s surname)
  4. Sworn Attestation to be executed by the mother (only of AUSF is NOT executed by mother) (https://philembassy.org.nz/download#civilregistry) – 4 original, to be notarized by the embassy or consulate
  5. Affidavit of Illegitimacy (if the child is not acknowledged by father) – 4 originals, to be notarized by the embassy or consulate
  6. Certificate of Recognition to be executed by the embassy – 4 original
  7. Please call the Embassy at 04-8903741 for fees regarding additional documentary requirement for unmarried parents

Requirements for Report of Marriage

  1. Report of Marriage form - 4 original
  2. Marriage Certificate or Particulars of Marriage – original and 4 copies
  3. Birth certificate of both husband and wife (PSA or NSO for Filipino Nationals) – original and 4 copies
  4. Passport (husband and wife) – 4 copies
  5. Affidavit of Late Registration of Marriage (stamped by public notary) – original and 3 copies if marriage was contracted more than a year ago
  6. Payment should be cash/bank check (payable to Philippine embassy) or please refer to our consular fees (https://philembassy.org.nz/consular/schedule-of-fees)
  7. Prepaid courier bag signature required
  8. In case of death of spouse, PSA(NSO) death certificate
  9. In case of annulment, marriage contract with annotation of nullity of marriage
  10. Any documents that may be required by the Philippine Embassy

Requirements for Report of Death

  1. Report of Death (form) - 3 original copies
  2. Death Certificate - original and 3 copies
  3. Medical certificate - original and 3 copies
  4. Certificate of No Infectious Disease – 3 copies
  5. If cremated, Certificate of Cremation – 3 copies
  6. Passport – 3 copies
  7. Payment should be cash/bank check (payable to Philippine embassy) or please refer to our consular fees (https://philembassy.org.nz/consular/schedule-of-fees)
  8. Prepaid courier bag signature required

Requirements for Consular Mortuary Certificate:

  1. Death Certificate - original and 1 copy
  2. Medical certificate - original and 1 copy
  3. Certificate of No Infectious Disease – 1 copy
  4. If cremated, Certificate of Cremation – 1 copy
  5. Passport – 1 copy
  6. Flight details
  7. Name and address of consignee
  8. Payment should be cash/bank check (payable to Philippine embassy) or please refer to our consular fees (https://philembassy.org.nz/consular/schedule-of-fees)
  9. Prepaid courier bag signature required
